Please complete this form to request an appointment for your exam. Appointment confirmation & payment instructions will be sent through email, please read all email correspondence carefully to ensure you have completed all the steps & can meet the technical requirements if you are taking this exam remotely.

You will need to complete the free MCCCD admissions process before requesting an appointment for any of the HESI exams; if you need to apply, follow the steps below, then return to this form:
   1.  Navigate to Become a Student
   2.  Select "New Student" or "Returning Student" 
   3.  Complete the application process within 48 hours, write down your Student ID Number, and MEID - return to              this page to make an appointment

Services are available Monday-Friday. Appointments are limited and scheduled first-come, first-serve based on student requests. You are strongly encouraged to schedule your appointment as soon as possible to ensure the best availability. Allow at least one full business day (24 hrs Monday-Friday) to process your request; requests submitted after 3pm on Friday will be processed no later than 5pm on the following Monday - please plan accordingly.

Policies & Procedures

  1. Please allow at least two business days to process your appointment request.
  2. Students are required to check in at least 15 minutes before their appointment; students who arrive late may be required to reschedule their appointment.
  3. Students must present a current, valid photo ID (state, federal, or college) and a printed/digital copy of their appointment confirmation email when they check-in for their exam.
  4. Students must store all personal belongings (hats, scarves, phones, watches, backpacks, etc.) in the Testing Services lockers while they complete their exams. Electronic devices (phones, laptops, tablets, etc.) must be turned off and stored during exams.
  5. Head coverings are prohibited in Testing Services; limited exceptions exist for religious or medical purposes. If this applies to you, please inform the proctor. All head coverings may be subject to inspection.
  6. Food/beverages are not allowed in the testing room.
  7. Restroom breaks are not allowed during exams.
  8. Children are not allowed in the testing room, nor may children (under 18) be left unattended while you complete your exam.
  9. All testing is subject to video and audio recording and surveillance.
