Employee Instructions for Work-Related Injuries
- If life threatening injury, contact 911, immediately.
- Report injuries to supervisor immediately; but no later than the end of the shift, no matter how minor the incident appears to be.
- Contact Tristar Nurse Triage at 1- 833-516-1684.
- Provide the nurse with your legal name.
- You will be asked for your mailing address, phone number, and email address.
- You will be asked for your date of birth and social security number.
- You will be asked questions about how the accident occurred and the specifics of the injury.
- Follow the nurse’s instructions; either self-care or seek treatment.
- If treatment is needed, Tristar will direct you to the nearest clinic/facility. Tell the medical facility that you are an employee of Maricopa Community Colleges and that we are self-insured, and our claims are managed by Tristar Risk Management.
- Provide a copy of work status (restrictions), if applicable to both the supervisor and Risk Management.
- Adhere to work status restrictions; applies to personal activities as well.
- Communicate and cooperate with your adjuster, supervisor, Risk Management, and medical professionals throughout the claims process.
- Schedule follow-up appointments so that they minimally impact your work schedule.
- Attend all appointments
- Keep the supervisor informed of work status, appointments, and overall progress.
- Provide documentation of follow-up appointments to supervisor and Safety & Risk Control Manager.
- Participate in the MTTD program.
Failure to follow any of these procedures may result in:
- Delay in treatment
- Denial of claim
- Out-of-pocket expenses
- Delay or interruption of wages
- Disciplinary action
