Student Club Facility Request Form

Clubs/Organizations are required to use this form to reserve a PC vehicle and/or a campus space at least two weeks prior to the scheduled event. Requests will be reviewed for approval by your Club Advisor and the Director of Student Life and Leadership. If this is a VEHICLE REQUEST ONLY please note "Vehicle Request ONLY" in the Event Name and Description box.

What will money raised be used for?

Co-Sponsored Event

All co-sponsored events require the attendance and coordination of a board approved faculty or staff member for the entire time the facilities are in use.  Please fill out the Facility Rental Fee Waiver Form if you believe this event qualifies for reduced or waived facility use fees.  This form must be turned in to the Event Scheduling Office for processing.  The co-sponsor must complete Facility Request Form and return it to the Events Scheduling Office no later than 60 days prior to the event.  Facilities and services cannot be guaranteed if this form is not completely filled out and turned in on time.  All fees must be paid with certified funds 5 working days prior to the event.  Please provide the name and contact information for the outside agency/group you are co-sponsoring using the space below.

A Certificate of Insurance in the amount of $1,000,000.00 listing Maricopa Community College District at 2411 W. 14th St., Tempe, AZ  85282, as the additional insured is required from all outside agencies/groups holding or participating in an event at PC.  The Certificate of Insurance must be received by the Phoenix College Event Scheduling Office no later than 10 working days prior to the event.

Food Sales or Service

Please contact Andrea Villarreal at the Culinary Café, 602.285.7901, or Stephen Shields with Chartwells, 602.285.7669, if this event requires catering services.  If food or refreshments will be sold/served, please provide the name of the group who will provide food service in the space below. All food sales & handling must be conducted in accordance with the AZ State Health Department guidelines.  Please see Student Life & Leadership for these requirements.
